Program: Senior Community Care (SCC)
Location: Wake & Chatham Counties, North Carolina

Schedule: Monday-Friday, 8 AM-5 PM
Travel: Required between assigned facilities during shift
Dress Code: Medical attire (no required scrub color)

Position Overview

The Senior Community Care (SCC) program is an integrated, longitudinal care model that supports high‑risk health plan members residing in facility settings. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will deliver high-quality, person‑centered care as part of an interdisciplinary team dedicated to improving clinical outcomes, reducing hospitalizations, and enhancing quality of life.

Primary Responsibilities
  • Conduct comprehensive clinical assessments for members residing in assigned facilities
  • Independently manage acute and chronic medical and behavioral health conditions
  • Ensure high‑quality, complete documentation including ICD‑10 coding
  • Communicate and collaborate with interdisciplinary care teams, including physicians, nursing staff, and facility leadership
  • Lead advanced illness and advanced care planning conversations
  • Provide education, counseling, and support to patients, caregivers, and facility staff
  • Participate in team-based meetings and case discussions as needed
  • Support continuity and coordination of care across multiple settings
Required Qualifications
  • Active and unrestricted NP or PA license in state of residence (or ability to obtain by start date)
Education Requirements:

Nurse Practitioner:

  • Graduate of an accredited MSN or DNP program
  • Board certification through AANP or ANCC in one of the following:
    • Family Nurse Practitioner (FNP)
    • Adult Nurse Practitioner
    • Gerontology Nurse Practitioner
    • Adult-Gerontology Acute Care Nurse Practitioner

Physician Assistant:

  • Graduate of an accredited PA program
  • Board certified through NCCPA
Additional Requirements:
  • Current and active DEA licensure/prescriptive authority
  • Ability to complete physical job functions, including lifting 30 lbs and performing on-site assessments in varied environments
  • Reliable transportation for travel between facilities
  • Ability to obtain a collaborative practice agreement, if required by state law
  • Full COVID‑19 vaccination (per federal, state, and client requirements)
  • Must pass pre-employment drug screening (UnitedHealth Group is a drug-free workplace)
Preferred Qualifications
  • 1+ year of clinical experience (long‑term care strongly preferred)
  • Experience with patients who have complex behavioral, social, or functional needs
  • Strong understanding of geriatrics, chronic illness, and end‑of‑life care
  • Experience conducting advanced illness discussions
  • Proficiency with EMR systems and electronic documentation
  • Excellent communication, time management, and interdisciplinary collaboration skills
