Position Description

Summary

The primary role of the Wound Care Specialist is to assess, manage, and treat patients with acute and chronic

wounds across a variety of care settings including clinics, skilled nursing facilities, and home health environments.

Key responsibilities include performing wound assessments, debridement procedures, and developing individualized

treatment plans in collaboration with interdisciplinary care teams. The Wound Care Specialist should approach every

patient encounter with clinical expertise, compassion, and integrity, ensuring evidence-based, timely, and effective care

aligned with the mission of improving outcomes and enhancing patient well-being.

  • Perform comprehensive physical examinations and wound evaluations on patients with acute and

chronic conditions.

  • Assess wound-related conditions, review comorbidities, and coordinate treatment plans with the

managing provider to support wound healing and overall health.

  • Perform wound debridement and dressing changes based on clinical necessity and best practice

standards.

  • Develop and implement individualized care plans in collaboration with the Medical Director and

interdisciplinary teams.

  • Accurately document all assessments, procedures, and communications in the EMR system.
  • Complete all clinical documentation within 24 hours of the patient encounter.
  • Provide patient and caregiver education regarding wound management, preventive care, and chronic

condition support.

  • Communicate effectively with primary care providers, specialists, and interdisciplinary teams regarding

changes in patient status.

  • Collaborate with medical assistants, clinical support staff, and regional leadership to maintain workflow

efficiency.

  • Participate in clinical meetings, training sessions, and quality improvement initiatives.
  • Serve as a patient advocate, ensuring access to necessary medical resources and follow-up care.
  • Provide compassionate, patient-focused education tailored to each individual's needs and

understanding.

  • Maintain compliance with all clinical, safety, and regulatory standards.
  • Support organizational initiatives that enhance patient outcomes, staff training, and operational

performance.

  • Maintain professional licensure and pursue continuing education in wound care and related specialties.
  • Maintains knowledge of current CMS documentation and treatment guidelines.
  • Perform other duties as assigned by leadership.

Qualification Guidelines

EXPERIENCE/TRAINING/EDUCATION

Required

Current, unrestricted Nurse Practitioner or Physician Assistant license in applicable state(s).

Clinical experience in wound care or a related specialty. Demonstrated proficiency in wound assessment, debridement, and plan-of-care development.

Desirable

Wound care certification (WCC, CWCN, or equivalent). Experience working in SNFs, home

health, or outpatient wound care settings. Familiarity with EMR platforms and mobile charting systems.

Knowledge/Skills/Abilities

Strong clinical judgment and ability to work independently in diverse settings.

Excellent verbal, written, and interpersonal communication skills.

Compassionate and patient-focused approach to care delivery.

Ability to manage mobile schedules, multitask, and adapt to changing priorities.

Commitment to professional growth, ongoing education, and evidence-based care practices.

Familiarity with regenerative medicine, wound healing technologies, and advanced treatment modalities.

Ability to utilize and adapt to emerging technologies to enhance care delivery and documentation

accuracy.

Work Environment

The work environment characteristics described here are representative of those an employee encounters

while performing the essential functions of this job. This position is primarily field-based, with travel

between clinical and community care sites. Frequent use of mobile technology and EMR systems is

required. The role may occasionally involve exposure to patients with infectious or complex conditions;

appropriate PPE will be provided.

While performing the duties of this job, the employee is regularly required to stand, walk, and travel

between patient care locations. The position may involve extended periods of standing, bending, or lifting

medical equipment and supplies up to 25 pounds. Reasonable accommodations may be made to enable
