As indicated in the New Year address, we have MORE strength and SUCCESS in numbers!!!  Please find attached are the vendor forms for YOUR drug representatives and a donation form for YOUR favorites places you frequent to ask of a donation for our gala event. We are planning door prizes and give aways, any donations will be utilized: gift certificates, specialty baskets etc....

Your help and assist is important to our success. Please print them out and copy for your offices, email them to those business cards tucked in your desk and reach across barriers to improve participation.
