On behalf of New Hampshire Family Voices:

NH Family Voices was approached by a provider last year with a request to investigate the creation of a resource for provider use. Basically, she wanted a central place to seek and to share information with other providers.

NH Family Voices created a survey last summer regarding this request, and sufficient interest was present to move forth.

As a result, we created a google group, named the NH Medical Home Resource Exchange.

We would like to invite anyone from your membership to join. Anyone who is interested in joining, or who would like additional information, can contact me for further details.
