This is a FREE evidenced-based program for your patients with any type of Chronic Illness. The aim of the program is to help participants establish competence in employing a TOOLBOX of coping strategies for optimizing their own self care. Topics such as: Sleep, Nutrition, Assertive Communication, Exercise and more are discussed in a peer group. Each week participants will set goals, and be held accountable for their progress. They will be guided in problem solving skills and priority setting. And they will learn from each other while providing support and encouragement.
