Greetings …… I am forwarding this on to the Colorado Rocky Mt. Chapter of NAPNAP members and ask that you consider completing the survey. As nurse practitioners, especially pediatric nurse practitioners in both primary care as well as specialty care, we are involved in coordination of care/care coordination at many different levels and in many different settings. We may not always call it care coordination but through the Pt. Centered Medical Home (PCMH) initiative and ACA nurse practitioner/advanced practice nurses are being recognized at the national level for these efforts. I would like to identify and document the work of PNP’s in assisting all children to connect with community resources as well as helping them receive the health care they need in Colorado. The Colorado Patient Navigator and Community Health Worker Collaborative hopes to learn more about the resources available to clients and activities underway in the area of patient navigation, community health work and care coordination. We hope the results of this survey will help to better coordinate services within communities through an understanding of what is needed to support this work and the current status of community health work, patient navigation and care coordination in Colorado to advance the field.
