I am currently a Doctor of Nursing Practice (DNP) student at Stony Brook University in New York and am conducting a research study entitled: "Nurse practitioner attitudes related to obesity in adults".

If you are an Adult (ANP), Adult-Geriatric (AGNP) or Family (FNP) Nurse Practitioner, licensed in New York State (NYS) and actively involved in the care of adult patients you are invited to voluntarily participate in this brief anonymous survey.

Participation includes completion of a simple 31item survey. There are no risks in taking this survey, and completion will take about 15 minutes of your time.

If you choose to participate, please click on the link at the end of this email, which will direct you to the online Survey Monkey's secure third party website. Please do not repeat participation in this survey if you have already done so through a previous email request.

The survey will have demographic questions, but will not contain any identifying information. All other questions will pertain to the study topic.
