Findings were reported to preceptor, and a plan of care was initiated in collaboration with preceptor; including referrals, diagnostic testing, and follow up care. Patient diagnoses and procedures include: diabetes, congestive heart failure, heart disease, pneumonia, bronchitis, COPD, vascular diseases, Otitis media, Otis externa, GERD, anemia, stroke musculoskeletal disorders, herpetic neuralgia, herpes zoster, herpes simplex, breast exams, and pelvic examinations.
(January 2011-April 2011): Conducted history and physicals, and focused interviews with at least ten patients per clinical day. Findings and plan of care reported to preceptor. Plan of care initiated with approval of preceptor. Plan of care included referrals, diagnostic testing, and follow up care. Patient populations include men and women of all ages. Minimal preceptor guidance required.

Greater than 20 patients seen per day with preceptor; six to seven patients assessed independently with findings reported to preceptors. Plan of care was initiated with collaboration with preceptor; including referrals, medications, diagnostics, and follow up are. Ages of patients ranged from newborn to 18 years of age. Common patient diagnoses include: Otitis media, Otitis externa, anemia, asthma, allergic rhinitis, sinusitis, bronchitis, pharyngitis, pyloric stenosis, contact dermatitis, seborrheic dermatitis. In addition, various infectious skin disorders respiratory, cardiac, gastrointestinal, musculoskeletal, neurological, and chronic conditions were encountered. Assessment included appropriate staging of growth and development.

(August 2010-December 2010): Conducted history and physicals and focused interviews. Physicals include pelvic examinations with Affirm and Pap smear testing. Greater than 25 patients seen per day; at least 10 patients assessed independently with findings reported to preceptor. Patient population includes women between the ages of 14 and older seeking prenatal or postpartum care. Plan of care was initiated independently, with approval by preceptor. Plan of care includes initiated of medications, referrals, diagnostics, and follow up care. Gynecological disorders that were diagnosed and treated were Herpes Simplex Virus, HIV, yeast infection, bacterial vaginosis, Chlamydia, trichomonis, gonorrhea, urinary tract infections, mastitis, mastalgia, fibroadenoma, and fibrocystic changes. Assessment included appropriate fetal growth. Cesarean sections, tubal ligations, and vaginal deliveries frequently observed.

included monitoring post operative patients and pacemaker/defibrillator interrogation.
Approximately six to ten patients seen per day, either as inpatient or outpatient. Patient
population includes men and women over the age of 18. Plan of care initiated with the guidance
of preceptor. Plan of care included initiated of medications, diagnostic testing, and follow up
care. Cardiac conditions that were diagnosed and treated include syncope, arrhythmias,
congestive heart failure, vulvular disorders, and coronary artery disease. Clinical time was spent
observing pacemaker/defibrillator placement, nuclear stress test imaging, echocardiogram
studies, electrophysiology studies, and bypass surgery.

(January 2011-April 2011): Conducted focused interviews and assessments in an acute care
setting. Approximately twenty patients seen per day with minimal preceptor supervision.
Findings, plan of care, and follow up were reviewed with preceptor before initiation. Patient
populations included men, women, and children across the lifespan, with various cultural and
ethnic backgrounds. Various conditions treated within each body system, including traumatic
injuries requiring suturing. Majority of clinical time was spent promptly identifying a differential
diagnoses and providing efficient care in an emergent setting.
