The transition from pediatric to adult care is a crucial milestone for individuals with cystic fibrosis (CF), as well as their caregivers and care teams. Supporting a seamless and gradual transfer of responsibilities to the person with CF is vital for achieving positive health outcomes.

In this video, expert faculty share lessons from the field and explore best practices and multi-disciplinary strategies to facilitate successful transitions, with an emphasis on shared decision-making.

As the Director of the Toronto Adult Cystic Fibrosis (CF) Clinic at St. Michael's Hospital, Dr. Tullis is responsible for the care of 600 adults with CF. Dr. Tullis is a Professor of Medicine at the University of Toronto, the Respirology Division Head at St. Michael's Hospital, and an Associate Scientist in the Li Ka Shing Knowledge Institute. She was the CF Canada Chair in Adult Cystic Fibrosis from 2014-2019. Dr. Tullis has published over 180 manuscripts and has lectured nationally and internationally on issues related to CF in adults. She has had 30 years of experience with CF clinical care. She has trained 25 CF fellows who are now working in CF clinics around the world. In 2012, she was awarded the Queen Elizabeth II Diamond Jubilee Medal to recognise her contributions to the care of people with cystic fibrosis in Canada. In 2017 she received the Dr Douglas Crozier Award from Cystic Fibrosis Canada in recognition of significant contribution and outstanding dedication to the fight against cystic fibrosis and the Prix d’Excellence-Specialist of the Year award from the Royal College of Physicians and Surgeons of Canada. In 2022, she was awarded the Jeffrey C Lozon Physician Leadership Award, at St Michael's Hospital.

Noah Lechtzin
Noah Lechtzin MD, MHS

Dr. Lechtzin is a pulmonary and critical care medicine physician. He directs the Adult CF program and the neuromuscular pulmonary clinic at Johns Hopkins and is an expert in the treatment of cystic fibrosis, pulmonary complications of neuromuscular disease and the care of critically ill patients in the Medical ICU. He has joint appointments in Epidemiology and Neurology. He is the co-director of the Johns Hopkins CF Therapeutics Development Network site.
